Save time in line!  Complete paperwork for personal training prior to arriving at the Cashier's Window of the Ramsey Student Center. 

The following forms must be completed in order to sign up for personal training:

Personal Training Waiver

Medical Screening Form - Males over 45 years of age or older, females 55 years of age or older or those who have one or more risk factors identified on this form are required to complete the Physician Referral Form. 

Physician Referral Form - This form must be completed by physician and brought to first meeting with the personal trainer. 

Don't forget to Meet the Trainers to choose the personal trainer you would like to work with.
